Please pre-register your child, and complete your volunteer application online at eayso.org and bring 3 copies of each with you to one of the above scheduled dates to complete the registration process and select your volunteer role.

***Special note for U5 Program:
UNTIL you, the parent(s), is/are a registered and approved as an AYSO volunteer and have completed the required AYSO Safe Haven Certification Course (and has been verified prior to the first session), your child will NOT be able to participate.

Print & bring 3 copies of the Player Registration Form,
and
2 copies of your Volunteer Application Form
to one of our three registration dates on either May 17, 18, or 31st.
Birth Cerificates required for first time players and a Government Issued ID is required of every Volunteer. (These will not be kept by the Region, but must be presented to sign up).
